Guide to filling out the ESTA application

Examples of filling out the ESTA application are provided below.
Before starting to fill out the form, make sure you have your passport and credit card at hand.


1.Start the application process

Click on "Apply for ESTA here" to begin the application process.

2.Enter applicant information

Enter your family and given names in Roman alphabet capital letters, exactly as shown on your passport. If you have any other name or alias, check the appropriate box and enter your other name(s). If you are unsure which city you were born in, you may leave "City of Birth" blank.

3.Enter the information from your passport

Enter the number of your passport issued by your own country.
Be careful not to mix up the "Issuance Date" and "Expiration Date."

4.Answer if you have ever been issued a passport or national identity card for travel by any other country.

If you have been issued a passport or national identity card for travel by any country other than the one indicated "Country of Citizenship" under the applicant information, select "Yes" and enter the issuing country, document type, document number, and expiration date.

5.Answer the question on citizenship/nationality

If you are now, or have been in the past, a national or citizen of a country other than your country of citizenship, select "Yes" and answer the corresponding questions.

6.Enter your parents' names

Enter your parents' names in Roman alphabet capital letters. You may leave one or more of these spaces blank if you do not know the name(s) of one or both of your parents. You may enter the name(s) of your biological parents, adoptive parents, step-parents, or guardians.

7.Enter your contact information

Enter your email address and telephone number carefully, to make sure that they are correct. If you want to register emergency contact information, select "Yes" and enter the information. After you complete the application process, an email receipt for your application and email notification of your travel authorization will be sent to the email address you entered here. Also make sure to avoid the receipt email being sorted to the SPAM file when using a webmail or mobile email address.

8.Enter the name on the credit card used for payment

After you enter the applicant information, please make payment for the ESTA application service. The following payment methods are available: VISA, MasterCard, JCB, Diners Club, American Express, or PayPal.

9.Enter your home address

Enter your home address using the Roman alphabet. Make sure to enter the address correctly.

10.Employer information

Select "Yes" if you want to register employer information, and then enter them (employer name, employer address, employer telephone number, job title). You may enter past or present employer information. This information is one way of demonstrating that you do not intend to immigrate to the U.S.

11.Travel information

Select "Yes" if you are changing to a connecting flight departing the U.S. and "No" if you will stay in the U.S. If you selected "No" and you know the details such as where you will stay in the U.S., answer the following questions (name, address, building, unit number, and telephone number of where you will stay).

12.Other questions

Read all nine questions carefully and answer "Yes" or "No." There is a very high likelihood that your ESTA travel authorization will be refused if you answer "Yes" to even one of these questions. If your ESTA is refused, you will need to obtain a nonimmigrant visa from the U.S. embassy. Be sure to read the questions carefully before you answer them.

13.Consent to ESTA application

This is the final question on the ESTA application. Read both of the consent statements closely and check the boxes if there are no errors in the content of your application. After checking the consent items, click on "Check Information Before Sending" below.

14.Check Application Information page

On this page, double-check the application information you have entered to make sure that there are no typos or mistakes, and if the information is correct, click on "Send Above Information. Pay close attention especially when checking your name, passport number, and date of birth for any errors. This completes your entry of information to the ESTA application.


15.Payment of the application fee

Have your credit card ready and enter the card information on the payment card entry screen. You will be charged the application fee of 7,236 JPY (including tax), which includes the application fee of $14 charged by U.S. Customs and Border Protection.


16.Application complete

Your entry procedures are complete when the message "Application and screening in process" appears.


17.ESTA application receipt email

The ESTA application receipt email will be sent to the email address you registered during the application process. Note that it may take up to 72 hours to complete the application and screening process. Also make sure to avoid the receipt email being sorted to the SPAM file when using a webmail or mobile email address.


18.Notification of travel authorization

Your ESTA application is complete when you receive the notification of travel authorization at the email address you registered. We recommend that you save the ESTA authorization information (application number and expiration date) shown in the email as a receipt for the information received from the U.S. Department of Homeland Security.
